Benefits - Personal Data Changes
Members should advise us of such changes as: address,
telephone number, name, or social security number change.
These changes must be submitted in writing.
Documents for Address Change
- An address change form or signed written letter
accompanied with a copy of a photo ID
- Only the retiree, or someone who has a Power of
Attorney, or a court-ordered ordered guardianship may
make an address change
- The original Power of Attorney or court-ordered
guardianship must be presented to GERS; (we will make a
copy for our records)
Documents for Name Change
Anyone changing their name should attach the appropriate
document such as: marriage certificate, birth certificate,
or other legal documents.
Documents for Social Security Number Change
An authorization letter from the SSA or a copy of the
social security card and a picture ID is required to change
a social security number.
